About Us:
Montrose Adult Day Health Care Center is a licensed Adult Day Health Care (ADHC) center dedicated to providing culturally competent, compassionate, and comprehensive care to our diverse aging population. We are currently seeking a dedicated Social Worker Assistant who is fluent in both Korean and English to support our growing Korean-speaking participant population.
Position Summary:
The Social Worker Assistant will support the Social Worker in delivering social services to program participants, particularly those from the Korean-speaking community. The ideal candidate will help facilitate communication, support care planning, conduct assessments, and assist with case management and community resource linkage in both Korean and English.
Key Responsibilities:

Assist the Social Worker in conducting participant intakes, psychosocial assessments, and care plan development.

Provide translation and interpretation services between staff and Korean-speaking participants/families.

Support participants in accessing resources, benefits, and services in the community.

Maintain accurate documentation in participant records per Title 22 and ADHC standards.

Help coordinate group activities and provide individual support under supervision.

Serve as a liaison between the center, families, and community agencies.
Qualifications:

High school diploma or equivalent; associate degree or relevant coursework in social services preferred.

Minimum 1 year of experience working with elderly or disabled populations.

Fluency in Korean and English (verbal and written) is required.

Strong interpersonal, organizational, and communication skills.

Familiarity with ADHC or similar community-based care programs is a plus.
Preferred Qualifications:

Experience in a multicultural setting.

Knowledge of Korean culture and traditions as they relate to aging and caregiving.
