Location: Buffalo Grove, IL (On-site)
Type: Full-Time, MondayFriday, 8:00 a.m.5:00 p.m. CT
Salary: $85,000$95,000 annually (based on experience) + eligible annual incentive bonus

We are seeking an experienced Clinical Manager to join a high-performing home health team in Buffalo Grove, IL. This is a permanent leadership opportunity within a 5-star rated branch serving a census of 420 patients. Key Responsibilities

• Coordinate and oversee all direct patient care services provided by the clinical team.

• Manage daily clinical operations, ensuring high-quality and compliant care delivery.

• Assign and guide caregivers, providing clinical oversight and mentorship.

• Ensure compliance with Medicare, CMS, and third-party payer requirements.

• Monitor documentation and implement quality improvement measures.

• Provide staff education and training.

• Participate in branch growth initiatives and act as Branch Director in their absence.

Qualifications

• Required:

  • Active RN license in the state of Illinois

  • Graduate of an accredited nursing program

  • Minimum 2 years of RN experience, including at least 1 year in home health management

  • Strong knowledge of OASIS (certification preferred)

  • Current CPR certification

  • Valid drivers license, auto insurance, and reliable transportation

• Preferred:

  • Proficiency in Homecare Homebase (HCHB)

  • Knowledge of CMS PDGM billing

  • Proven management and leadership skills in a home health setting
