Type: Full-Time, MondayFriday, 8:00 a.m.5:00 p.m. CT
Salary: $85,000$95,000 annually (based on experience) + eligible annual incentive bonus
We are seeking an experienced Clinical Manager to join a high-performing home health team in Buffalo Grove, IL. This is a permanent leadership opportunity within a 5-star rated branch serving a census of 420 patients. Key Responsibilities

Coordinate and oversee all direct patient care services provided by the clinical team.

Manage daily clinical operations, ensuring high-quality and compliant care delivery.

Assign and guide caregivers, providing clinical oversight and mentorship.

Ensure compliance with Medicare, CMS, and third-party payer requirements.

Monitor documentation and implement quality improvement measures.

Provide staff education and training.

Participate in branch growth initiatives and act as Branch Director in their absence.
Qualifications

Required:
-
Active RN license in the state of Illinois

Graduate of an accredited nursing program

Minimum 2 years of RN experience, including at least 1 year in home health management

Strong knowledge of OASIS (certification preferred)

Current CPR certification

Valid drivers license, auto insurance, and reliable transportation

Preferred:

Proficiency in Homecare Homebase (HCHB)

Knowledge of CMS PDGM billing

Proven management and leadership skills in a home health setting
