Our organization operates Skilled Nursing Facilities (SNFs) that offer:

  • Long-term care for residents in need of ongoing support.
  • Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury.
  • Collaborative care through a team-oriented approach involving nursing, therapy, and medical professionals.

The patient demographic may vary slightly across different facilities. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life within a nurturing, patient-focused setting.

Key Responsibilities

  • Deliver direct rehabilitation services in occupational therapy to patients based on physician referrals, adhering to all relevant laws, regulations, and organizational standards.
  • Engage in community health initiatives and projects as appropriate.
  • Utilize Microsoft Word, Excel, and email proficiently.
  • Maintain knowledge of practices, procedures, and regulations governing operations in post-acute care facilities.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
  • Ensure confidentiality of all proprietary and sensitive information.
  • Comply with organizational policies, including those related to harassment and compliance.
  • Exhibit integrity and professionalism by adhering to the organization's Code of Conduct and completing all required compliance training.
  • Supervise and assess the care provided by Occupational Therapy Assistants (OTAs), Rehabilitation Aides, and students.
  • Utilize therapy software accurately and effectively.
  • Provide exceptional customer service and maintain a positive attitude towards patients.
  • Read, write, speak, and understand the English language proficiently.

Qualifications

  • Graduate of an accredited occupational therapy program (BSOT or MSOT).
