Job DescriptionSalary: $30 per hour

Position Summary:Visin y Compromiso is committed to enhancing community health through proactive outreach and education. We are seeking a dedicated Lead Promotora Comunitaria / Promotor Comunitario Community Health Workerto provide essential health services and support to our community members. Under the direct supervision of the Project Coordinator, the Community Health Worker (CHW) will conduct CHW Services: preventive health services to prevent disease, disability, and other health conditions or their progression to prolong life and well-being awareness activities to increase overall access to care for community members. Services may include activities such as health education classes on various topics to address barriers to physical health and mental health.

Location: In-person in San Joaquin County (some administrative work from home or an approved remote office)

Position Status: Regular, Full Time, Hourly

Effective Date: August 2025

Key Responsibilities:

Community Engagement

• Conduct preventive health services to address chronic conditions and promote overall well-being.
• Engage with community members through outreach activities, health education classes, and resource navigation.

Support and Advocacy

• Assist community members in accessing health services and provide individual support.
• Participate in developing personalized care plans and follow up to ensure ongoing engagement.

Planning and Coordination

• Collaborate with clinical partners and maintain constant communication with the Project Coordinator.
• Coordinate outreach events and workshops to increase community awareness and access to care.

Required Qualifications:

• Minimum of 2 years of experience working with bilingual Latino communities and conducting outreach.
• Field experience in community health awareness and database navigation.
• Bilingual in English and Spanish.
• Ability to create and deliver health education curricula on various topics.
• Ability to do screening & assessment that will assist in providing services that do not require a license and connect members to appropriate services and community resources to improve health.
• Experience with navigation of social and state services.
• Knowledge of Medi-Cal programs & Public health care system.
• Experience with phone outreach & home visitation modalities.
• Experience with support groups and advocacy.
• Availability to work the scheduled hours as required, which may include evenings, weekends, and some holidays.
• Computer skills for the purpose of data entry during enrollment events, sending and receiving email, and calendar scheduling.
• Comfort using and learning new technology.
• Proficient with report submissions and technology basics.
• Proficiency in Google Suite, Microsoft Office (Excel, Word, PowerPoint), Google Chrome or other widely used and supported Internet browsers, and Zoom or telecommunication software

Physical Demands:

• Ability to move about in various environments up to 75% of the time.
• Must be able to remain stationary for up to 50% of the time and lift outreach equipment weighing up to 20 pounds.
• Frequent outdoor work and interaction with community members.

Work Environment:

• Primarily field-based with travel to community locations, including door-to-door visits and public meetings.
• Occasional remote work from an approved home office.
