Are you a passionate, experienced nurse leader who thrives in both the clinical and operational world? We're looking for a Clinical Operations Manager to lead our Hospice and Skilled Home Health programs, someone who brings heart to end-of-life care and rigor to daily operations.

What You'll Do
Lead Clinical Operations
- Oversee day-to-day clinical operations for both hospice and skilled home health programs
- Provide hands-on guidance and support to nursing, therapy, aide, and social work staff
- Monitor staffing, caseloads, and scheduling to ensure continuity and quality of care
- Collaborate with the Administrator/Executive Director on strategic goals
Drive Quality & Compliance
- Ensure compliance with CMS Conditions of Participation (CoPs) for both service lines
- Lead survey readiness, internal audits, and corrective action plans
- Audit clinical documentation - including OASIS assessments and hospice plan of care updates - for accuracy and regulatory compliance
- Monitor quality metrics and drive performance improvement initiatives
Manage Intake, Admissions & Start of Care
- Oversee clinical aspects of intake and admissions for hospice and home health referrals
- Review referrals for medical necessity, eligibility, and payer requirements (Medicare, Medicaid, managed care)
- Ensure timely, compliant Start of Care visits and smooth patient transitions
Hospice Program Oversight
- Lead interdisciplinary team (IDT) meetings and comprehensive care planning
- Support symptom management, goals-of-care conversations, and end-of-life care delivery
- Oversee bereavement and volunteer coordination programs
Home Health Program Oversight
- Oversee OASIS completion, accuracy, and submission timelines
- Monitor visit utilization, episode management, and value-based care outcomes
- Ensure therapy and nursing plans of care are individualized and outcome-driven
Develop & Inspire Your Team
- Recruit, onboard, mentor, and evaluate clinical staff across both programs
- Oversee competency assessments, orientation, and ongoing clinical education
- Foster a culture of accountability, teamwork, and continuous improvement
What You Bring
Required:
- Current, unrestricted Registered Nurse (RN) license in the state of practice
- 3–5+ years of clinical experience in skilled home health and/or hospice
- Demonstrated leadership or supervisory experience in a clinical setting
- Strong knowledge of CMS CoPs, Medicare/Medicaid regulations, and clinical documentation standards
- Proficiency with Electronic Medical Records (EMR) systems
- Excellent communication, organizational, and problem-solving skills
Preferred:
- BSN required; Master's degree a plus
- Experience with OASIS documentation, QAPI programs, or regulatory survey preparation
- CHPN (Certified Hospice and Palliative Nurse) or CRRN certification
- Experience managing dual or multi-program clinical operations
Work Environment
This is a hybrid field and office role. You'll work in patient homes, care facilities, and agency offices - and will travel locally as needed. Standard clinical protective equipment is provided.
