- Schedule: 5x8 Hours | 40 Hours/Week
Position Overview
"Navitas Healthcare, LLC" is seeking an experienced RN Case Manager to join a fast-paced acute care facility. The role focuses on care coordination, utilization review, and discharge planning while ensuring high-quality patient outcomes.
Key Responsibilities
- Coordinate care plans and discharge processes
- Perform utilization review using InterQual criteria
- Manage caseload (~1:20 ratio)
- Collaborate with interdisciplinary teams
- Maintain accurate documentation in Epic EMR
Required Qualifications
- Active California RN License
- Recent acute care case management experience (within 6 months)
- Epic EMR experience (Required)
- InterQual knowledge (Required)
- First-time travelers not accepted
