EMPLOYMENT TYPE: Temporary 1.0 FTE (80 hours per pay period) to last approximately 6 months

What you'll do:
This role directly supports the clinic patient visits and hospital orders derived from internal medicine/wound, ostomy, continence (WOC) Provider. The role provides rooming for clinic patients in this provider's schedule and provides wound care and dressings per provider's orders and direction. The wound care navigator role assists with patient education, performs dressing changes, executes wound/ostomy supply management for both the clinic and hospital, and orders wound/ostomy supplies at individual patient visits. The RN WOC Navigator incorporates current research and best practice guidelines when developing a plan of care for wound, ostomy, and continence care.
- Follows the RN process for nursing assessment, diagnosis, planning, implementation of care, and evaluation of patient progress to function in a full scope RN role to support providers in the clinic.
- Acts as a resource to LPN, CMA, and other patient care staff in assigned unit in accordance with the Minnesota Nurse Practice Act (NPA).
- Performs rounds on inpatient units with provider to assist in tasks including, but not limited to, WOC care, repositioning, consultation, education, and order follow-up.
- Ensures there is timely follow up in the wound clinic after hospital discharge.
- Calls patients to ensure wound/ostomy supplies were received and answer any wound care questions or concerns.
- Assists in ordering of pressure reducing and support surfaces received from vendors.
- Measures patient for compression garments in addition to ordering and stocking compression garment supplies in clinic and processing/submitting the billing of the garments.
- Ensure referral specialty appointments (vascular, plastic surgeon, burn specialist, etc.) are made and attended.
- Support and troubleshoot clinic and hospital wound/ostomy/continence/lymphedema needs.
- Incorporate management of diabetes for patients with diabetic wounds.
- Address additional BPAs (best practice advisory) at wound/ostomy care outpatient visits as necessary for all wound/ostomy patient.
- Works collaboratively with physicians/providers and nursing personnel to triage patients over the phone, directing them appropriately with best clinical judgement.
- Works collaboratively with clinic personnel/departments and hospital personnel/departments to support smooth patient transitions.
- Performs pre-visit planning work to obtain reason for visit, pharmacy, chief complaint, etc. prior to visit to ensure in-clinic appointment time is well utilized with provider.
- Other duties in support of unit patient's plan of care and consistent with full scope RN practice in accordance with Minnesota Nurse Practice Act.
Qualifications:
- Has a current RN license to practice nursing in the state of Minnesota.
- Demonstrated knowledge of the Minnesota Nurse Practice Act and the Scope of Ambulatory Care Nursing
- Clinic experience preferred
- One year of wound care experience required
- Has current Wound Care Certification as NAWCO or WOCN.
- Demonstrates clinical competence in providing direct patient care and patient and family education as well as assisting the physician in the delivery of patient care
- Has current BLS certification (or is willing to obtain within two months of employment.)
- Demonstrated ability to manage the complex needs of a family medicine practice
- Demonstrated competence in managing patients using Patient Care Protocols
- Ability to manage change in a positive patient centered manner
- Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, and mission
- Maintains reliable and consistent attendance
