Job DescriptionNurse Practitioner / Physician Assistant – Wound Care Job Summary We are seeking a Nurse Practitioner or Physician Assistant to provide mobile, evidence-based wound care services. This role focuses on assessing, treating, and managing wounds while ensuring high-quality patient care, compliance, and strong care coordination. Key Responsibilities

<li>&nbsp;Perform comprehensive wound assessments and develop treatment plans&nbsp;</li>
<li>&nbsp;Provide weekly patient visits (or as clinically indicated)&nbsp;</li>
<li>&nbsp;Prescribe treatments and manage wound-related orders within scope&nbsp;</li>
<li>&nbsp;Document all patient care accurately and timely (same-day)&nbsp;</li>
<li>&nbsp;Coordinate care with agencies, physicians, and internal teams&nbsp;</li>
<li>&nbsp;Escalate care when clinically appropriate&nbsp;</li>
</ul>
Requirements 
<ul>
<li>&nbsp;Active NP or PA license (unrestricted)&nbsp;</li>
<li>&nbsp;Strong clinical judgment and communication skills&nbsp;</li>
<li>&nbsp;Ability to work independently and collaboratively&nbsp;</li>
<li>&nbsp;Willingness to travel locally to patient homes/facilities&nbsp;</li>
</ul>
Schedule &amp; Travel 
<ul>
<li>&nbsp;Full-Time: Monday–Friday, 8:00 AM – 5:00 PM&nbsp;</li>
<li>&nbsp;Part-Time / Per Diem: Flexible based on patient volume&nbsp;</li>
<li>&nbsp;Travel required (up to ~55 miles)&nbsp;</li>
<li>&nbsp;No on-call required&nbsp;</li>
