Job Responsibilities
- Perform comprehensive patient evaluations and assessments to identify physical therapy needs and functional limitations
- Design and implement individualized treatment plans focused on improving patient mobility, strength, balance, and overall physical function
- Deliver evidence-based therapeutic interventions, manual therapy techniques, and rehabilitative exercises tailored to each patient's goals
- Provide patient and caregiver education on home exercise programs, adaptive equipment, and fall prevention strategies
- Collaborate with physicians, nurses, occupational therapists, and other interdisciplinary team members to ensure coordinated, comprehensive care
- Maintain accurate and timely clinical documentation in compliance with Medicare, Medicaid, and insurance requirements
- Monitor patient progress regularly and adjust treatment protocols to maximize functional outcomes
- Conduct home safety assessments and recommend environmental modifications as needed
Job Requirements
- Current, unrestricted Physical Therapist license in the state of practice
- Graduation from an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate degree in Physical Therapy)
- Home health physical therapy experience strongly preferred
- Valid driver's license and reliable transportation for home visits throughout the service area
- Excellent assessment, clinical reasoning, and problem-solving skills
- Strong verbal and written communication abilities to interact effectively with patients, families, and healthcare team members
- Proficiency with electronic medical records (EMR) and documentation systems
- CPR certification required
- Ability to work independently while managing a varied caseload and flexible schedule
