Under the guidance of the primary care operations team, the Primary Care Nurse collaborates closely with our Primary Care Providers to deliver safe, timely, effective, efficient, equitable, and patient-centered care in a structured manner. This role involves partnering with patients, healthcare providers, and other clinical team members to enhance quality outcomes for a designated patient panel. Responsibilities will be carried out in alignment with our mission, values, and service standards.
Position Overview:
Primary Care RN or LPN - Family Medicine Clinic
Work Schedule: Monday to Friday
No weekends or holidays!
**Completion of three months of on-site training is required before transitioning to a hybrid work-from-home schedule; availability for monthly clinic meetings and 1-2 on-site visits per week is also necessary.**
The Primary Care Nurse position is a newly established role within our clinics. In this capacity, the nurse will closely collaborate with Providers and manage triage calls and messages for the clinic. After training, there will be an opportunity to work in a hybrid model.
LPN Qualifications:
Education: Diploma in Practical Nursing
Licensure: Current LPN license
Experience: Preferred 2-3 years of relevant experience
RN Qualifications:
Education: Associate of Applied Science in Registered Nursing, graduation from an accredited nursing program, a bachelor’s degree in nursing (BSN), or 4 years of equivalent experience in lieu of a bachelor’s degree
Licensure: Current RN license
Experience: Preferred 2-3 years of relevant experience
