<ul>
<li>Masters of Social Work from accredited school</li>
<li>Current state license as a social worker</li>
<li>Minimum of 2 years experience as a social worker in a clinical setting, preferrable Hospice or healthcare setting</li>
<li>Reliable transportation required</li>
</ul>
<p>Responsibilities</p>
<ul>
<li>Provide psycho-social, emotional, and spiritual support to patients and families</li>
<li>Collaborate with IDT for better quality of care</li>
<li>Perform and provide updated comprehensive care plans which reflect current problems, goals, and interventions</li>
<li>Interact with facilities and provide paperwork at completion of visits</li>
<li>Explain Hospice services and medicare benefits to patients and families; Obtain informed consents and election of benefits documents as requested</li>
<li>Facilitate long term care placement for respite</li>
