SUPPORTED LIVING SPECIALIST
Qualifications: Minimum - High School graduate or GED.
Preferred - Experience working in Human Services or a related field.
Supervision Received: Supported Living Director
Supervision Given: None
Conditions of Employment: Tuesday - Saturday, up to 40 hours per week. Daytime schedule with occasional evenings, early mornings, weekends, and holidays. Some on-call. Involves transporting clients to/from scheduled activities. Must have own vehicle and be willing to occasionally use for transporting of clients. Any employee who operates a vehicle on behalf of the company must maintain a valid driver's license and insurability to include an acceptable driving record as per insurance company guidelines. A driving record check will be conducted at the time of hire and throughout employment. Must pass all applicable background checks. Non-Exempt. Must be able to move and stand for prolonged periods and perform physically demanding tasks at times. Must be computer literate or have a willingness to learn. Must have good oral and written communication skills.
Environmental Conditions: May be exposed to physical and/or verbal aggression. May be exposed to Blood Borne Pathogens and be required to use Personal Protective Equipment.
Probationary Period: Six (6) months
Essential Duties:
- Provide training, support and supervision to adults with disabilities with activities of daily living, as indicated by each client's service plan and/or contract agreement in the following areas: money management, shopping, meal planning and cooking, hygiene/grooming, household maintenance, community safety.
- Assess clients' skill level and assist with developing Personal Support Plans and/or contract agreements specific to each person's needs.
- Assist in implementing service plan goals at the direction of supervisor.
- Attend and participate in scheduled staff meetings.
- Plan and coordinate constructive/positive activities with clients.
- Arrange and/or provide transportation for clients to/from scheduled activities.
- Complete client incident reports as necessary, following incident management guidelines.
- Complete client contact notes, ensuring detailed and thorough documentation is provided.
- Assist clients, as needed, with accessing community resources such as Section 8, LIEAP, Social Security, Medicaid, SNAP.
- Communicate with family members and other service agencies when needed to coordinate client activities.
- Maintain required certifications and participate in scheduled training.
- Carry an emergency on-call phone when assigned and respond to client emergencies as needed.
- Overtime as required.
- Maintain positive working relationships with agency representatives, health/medical professionals, and family members of clients.
- Observe safety and health rules, regulations, and works according to standard procedures and practices outlined in personnel and safety manuals.
- Will not violate the confidentiality of any information relating to clients/eligible workers or any contract customers.
- Promote a positive image of COR Enterprises as outlined in the Personnel Policies under Code of Ethics.
