JOB DESCRIPTION
OT @ Kenosha - CH Home OT
Coverage will be for primarily Racine and Kenosha Counties. Expectation of 6 points/ day

About Facility:
o Home Health position that covers Kenosha and Racine Counties in Wisconsin
General Information:
o Beds: N/A
o Minimum Years of Experience? 3 years minimum OT experience. SNF, home health, or acute care, but preferrable to home health experience
o Will you accept a first-time traveler? Yes
Patient Types/Common Diagnoses:
o All adults, strictly home health
o Geriatrics, post-stroke, post-joint replacement, weakness, deconditioning, COPD, CHF, etc.
o Will work with RN and PT interdisciplinary teams.
o Morning team huddle via phone every morning
Case Load:
o 6 patients/ points per day Evaluation visits are 1.5, standard visit is 1
Required Licensure & Certifications:
o BLS, WI License
o Will accept a bachelor's degree in occupational therapy
o Vehicle, active driver's license, active auto insurance
Skills required:
o General OT treatment and evaluation skills focus on ADLs and adaptive equipment
Support within the Department:
o RN and PT interdisciplinary team
o RN Supervisor
o PT lead and PT manager
o Other OTs
Technology/Equipment:
o EMR: EPIC preferred.
Floating: If yes, where?
o Racine County and Kenosha County, WI. Most patients are within 20-30 minutes but could have occasional patient that could be close to an hour away (not common).
Orientation (facility and unit):
o First day of orientation will take place at National Avenue office in Milwaukee
o Will go through EPIC class
o Remainder of the week will be orientation with a preceptor
Scheduling:
o Weekend rotation: No
o On call? No
o Holiday Expectations: No
o Approval of RTO: to go through manager
o Shift times: around 0745-1600, Monday Friday.
o Schedule cycle: see above
Other notes:
o Scrub Color: Business casual or any color scrubs
o Parking: at patient houses
