Minimum Requirements

\n

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Wisconsin Physical Therapist license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Must have at least 12 months of home health experience within the past 3 years (client requirement).
  • Experience with HCHB (Homecare HomeBase) EMR preferred.
  • Ability to travel within a multi-county territory including Presque Isle, Manitowish Waters, Mercer, St. Germain, Eagle River, Land O' Lakes, and Phelps.
  • Current professional liability insurance as required by facility and state regulations.
  • Active TB screening and up-to-date immunizations per facility policy.
  • Clear background check and ability to meet any state-specific screening requirements.
  • Preferred: prior experience providing skilled home health therapy and documentation for Medicare plans of care.

Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Monday through Friday, 8-hour days. Schedule indicated as 8:00 AM–5:00 PM (client schedule: Monday–Friday 8am–5pm).
  • Positions Available: 1.
  • Length of Assignment: 13 weeks.
  • Call: No on-call required.
  • Territory: Travel required within Presque Isle, Manitowish Waters, Mercer, St. Germain, Eagle River, Land O' Lakes, and Phelps service area.
