Job Title: Director of Case Management
Location: Atlantis, Florida
Schedule: Full-Time | Days | No Weekends
Position Overview
A leading acute care hospital in Atlantis, Florida is seeking a dynamic and experienced Director of Case Management to oversee and coordinate all case management operations within the facility. This executive-level role is responsible for ensuring effective care coordination, regulatory compliance, and the integration of patient care and discharge planning processes across hospital departments and external agencies.
The Director will serve as the subject matter expert in case management and provide strategic leadership to drive operational excellence, quality outcomes, and optimal clinical resource utilization. This position requires strong executive presence, exceptional communication and presentation skills, and a proven ability to lead teams through change while achieving measurable results.
Responsibilities

Direct and evaluate all departmental operations, including case management models, staffing plans (skill mix & FTEs), onboarding, competencies, and use of information technology.

Ensure compliance with organizational policies, regulatory standards, and care coordination best practices.

Oversee core case management functions including utilization review, discharge planning, and documentation integrity.

Allocate resources effectively to meet productivity, financial, and quality goals.

Collaborate with executive leadership, including finance and division leaders, to achieve established operational and strategic objectives.

Monitor and analyze performance data, identify improvement opportunities, and implement sustainable performance improvement initiatives.

Foster strong working relationships with key stakeholders including medical staff leadership, nursing leadership, ancillary services, compliance, and legal teams.

Promote adherence to organizational mission, values, and code of conduct.

Lead change management initiatives while maintaining team engagement and trust.

Manage multiple priorities independently while demonstrating tact, diplomacy, and adaptability in complex situations.
Qualifications

Bachelor’s Degree (Required)

Master’s Degree in Nursing, Health Administration, or Business Administration (Preferred)

Active licensure in one of the following (Required):
-
Registered Nurse (RN)
-
Licensed Clinical Social Worker (LCSW)
-
Licensed Master Social Worker (LMSW)
-

Minimum 3+ years of acute care hospital case management experience (Required)

Minimum 2+ years of case management leadership experience (Preferred)

Demonstrated leadership, team-building, and change management expertise

Strong analytical, organizational, and communication skills

Proven track record of achieving performance and quality outcomes
