Position Summary:

Commonshare is seeking a compassionate and experienced Advanced Nurse Practitioner (ANP) to deliver comprehensive, patient-centered primary care services to elderly residents at Imperial Plaza, a senior living community in Richmond, Virginia. This on-site role will focus on providing accessible, high-quality healthcare tailored to the complex needs of a geriatric population, supporting aging in place and reducing unnecessary hospitalizations.

Responsibilities:

  • Perform physical exams, assessments, and routine primary care visits directly at the patient's residence within Imperial Plaza.
  • Diagnose and manage acute and chronic health conditions in older adults, including but not limited to diabetes, hypertension, COPD, and dementia.
  • Develop and implement personalized care plans in collaboration with patients, families, pharmacists, and other healthcare providers.
  • Monitor and adjust treatment plans based on patient outcomes and clinical guidelines.
  • Provide education to residents and their families about disease prevention, medication adherence, and wellness.
  • Coordinate care with Commonshare Pharmacy and other healthcare partners for medication management and follow-up.
  • Facilitate referrals to specialists and community resources when necessary.
  • Maintain detailed and compliant electronic health records and documentation.
  • Participate in interdisciplinary care team meetings and contribute to overall program development.

Qualifications:

  • Current license as a Nurse Practitioner (NP) in the state of Virginia.
  • Certification in Adult-Gerontology Primary Care or Family Nurse Practitioner (AGPCNP or FNP) strongly preferred.
  • Minimum of 2 years' experience in geriatric, internal medicine, or primary care settings.
  • Strong clinical assessment skills and comfort with managing multiple chronic conditions in older adults.
  • Excellent communication and interpersonal skills with a patient-centered approach.
  • Experience working in residential or long-term care settings is a plus.
