- Job Title: RN – Home Health
- Location: Wayne, Oakland, and Macomb Counties, Michigan
- Schedule: Monday–Friday, 8:00 AM – 4:30 PM + one Saturday per month
- Pay Rate: $45.00-48.00 an hour paid weekly
- Benefits: available to full-time employees after 90 days of employment
Position Overview:
Under the general supervision of a Nursing Supervisor, the Registered Nurse (RN) provides direct patient care in a home health setting. This role involves assessing patient needs, coordinating care with interdisciplinary teams, and ensuring high-quality, patient-centered services in the home environment.
Key Responsibilities:
- Provide direct nursing care to patients in their homes
- Assess patient conditions and develop individualized care plans
- Coordinate with allied health professionals including physical therapy, occupational therapy, nutrition, speech pathology, and social work
- Ensure continuity of care and integrated service delivery
- Maintain accurate and timely documentation
- Communicate effectively with patients, families, and healthcare teams
Schedule & Work Details:
- Approximately 6 patient visits per day
- Average travel: 40–60 miles per day
- Orientation: 2–4 weeks (combination of classroom and field training with preceptor)
Required Qualifications:
- Current Registered Nurse (RN) license in the State of Michigan
- BLS certification (American Heart Association required)
- Minimum of 1 year of Home Health experience
- At least 9 months of clinical nursing experience
- Valid Michigan driver’s license with a satisfactory driving record
- Reliable transportation and telephone
- Ability to lift, carry, and assist patients as needed
- Acute care experience
