  • Job Title: RN – Home Health
  • Location: Wayne, Oakland, and Macomb Counties, Michigan
  • Schedule: Monday–Friday, 8:00 AM – 4:30 PM + one Saturday per month
  • Pay Rate: $45.00-48.00 an hour paid weekly
  • Benefits: available to full-time employees after 90 days of employment

Position Overview:
Under the general supervision of a Nursing Supervisor, the Registered Nurse (RN) provides direct patient care in a home health setting. This role involves assessing patient needs, coordinating care with interdisciplinary teams, and ensuring high-quality, patient-centered services in the home environment.

Key Responsibilities:

  • Provide direct nursing care to patients in their homes
  • Assess patient conditions and develop individualized care plans
  • Coordinate with allied health professionals including physical therapy, occupational therapy, nutrition, speech pathology, and social work
  • Ensure continuity of care and integrated service delivery
  • Maintain accurate and timely documentation
  • Communicate effectively with patients, families, and healthcare teams

Schedule & Work Details:

  • Approximately 6 patient visits per day
  • Average travel: 40–60 miles per day
  • Orientation: 2–4 weeks (combination of classroom and field training with preceptor)

Required Qualifications:

  • Current Registered Nurse (RN) license in the State of Michigan
  • BLS certification (American Heart Association required)
  • Minimum of 1 year of Home Health experience
  • At least 9 months of clinical nursing experience
  • Valid Michigan driver’s license with a satisfactory driving record
  • Reliable transportation and telephone
  • Ability to lift, carry, and assist patients as needed
  • Acute care experience
