What You’ll Do
- Develop and manage personalized care plans to support optimal health outcomes
- Collaborate with physicians, therapists, and caregivers to ensure quality of care
- Provide education and emotional support to clients and families
- Keep accurate, timely documentation in compliance with state and agency standards
What You’ll Need
- Active Minnesota RN license
- At least 2 years of nursing experience, including 1 year in home care
- Valid driver’s license and auto insurance
- A heart for people and a desire to provide exceptional care
