Job DescriptionPhysical Therapist - Direct Hire Opportunity in Waverly, NY | Outpatient, Skilled Nursing & Assisted Living

Job Responsibilities

Develop and implement personalized physical therapy treatment plans based on physician orders and patient assessments
Conduct comprehensive evaluations and provide therapeutic interventions for geriatric, subacute, and adult patient populations
Document detailed progress notes, evaluations, and discharge summaries in accordance with regulatory standards
Collaborate with interdisciplinary healthcare teams including nursing staff, rehabilitation professionals, and physicians to optimize patient functional outcomes
Participate actively in care planning meetings and contribute clinical expertise to treatment strategies
Maintain accurate treatment documentation, monitor patient statistics, and manage supply requests
Lead or participate in staff training, continuing education, and professional development activities
Provide leadership support and supervise support staff in the absence of the Director of Physical Therapy

Job Requirements

Graduation from an accredited Physical Therapy program (DPT or equivalent degree)
Active New York State Physical Therapy license (required)
Strong clinical interest in treating geriatric patients, subacute rehabilitation, and adult populations
Excellent communication and interpersonal skills for effective patient and team collaboration
Demonstrated leadership qualities and ability to supervise rehabilitation support staff
Previous physical therapy experience preferred but not required - new graduates strongly encouraged to apply
Commitment to evidence-based practice and patient-centered care delivery
Ability to work effectively in outpatient, skilled nursing, and assisted living environments
