- LOOKING FOR 6/24 START - THIS IS FOR LOA COVERAGE
- Profiles go directly to the unit manager for consideration
- Need 2 sup refs (charge accepted) at submittal; 1 w/in 12 mo. & 1 w/in 3 years worked
- EMR HomeCare Home Base (HCHB) experience preferred. Home Health experience including OASIS required.
- 2 years experience in a healthcare setting required -- Community/home health experience preferred.
- Required certs: BLS & NPI # **Also needs a CA PT license**
- HH requires a current driver's license & proof of automobile insurance.
- Good oral and written communication skills; has good organizational skills.
- **Mileage is billable per GSA guidelines. Personal timesheets showing mileage must be signed by home health supervisors. There are meetings twice a month to have hard copies signed (if they do not see supervisors on a regular basis). They may also send photocopies of timesheets with mileage listed to the supervisors via email for approval. Timesheets need to be sent to MSP.Mailbox@medicalsolutions.com in order to bill.
- -Provides physical therapy services to patients/clients according to a written physician's plan of care. This may include, but is not limited to:
- -Assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
- -Evaluates home environment. Assesses for muscle strength, mobility, gait, ROM - potential for rehab. Directing physical therapy treatment.
- -Instructing patients/clients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for physical therapy.
- -Reporting the patient's/client's reaction to treatment or changes in condition to the physician.
- -Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with
- which they may work with a patient/client, as well as instructing them as to the goals of the physical therapy program for the patient/client by
- participating in case conferences.
- -May train patient/client in the use of prosthetic device.
- -Supervising homemaker/Home Health Aides when such personnel are participating in patient's/client's rehabilitation regime.
- -Identifies patient/client and family/caregiver needs for other home health services and refers as necessary.
- -Prepares and submits clinical and progress summaries based on the attainment of goals, observing, recording and reporting information on the
- patient's condition to the attending physician and in the patient's health record.
- -Prepares and submits a clinical progress summary based on the attainment of goals directed by agency policy.
- -Provides physical therapy consultation to home families/caregivers when indicated.
- -Provides inservice education programs for nursing agency personnel as needed.
- -Participates in peer consultation process.
- -Participates in and completes ongoing staff competency assessment.
- -Complies with and understands Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA and other regulations and licensure requirements as they pertain to their position.
- -Performs other duties as assigned and/or requested.
