Responsibilities
- Conduct intake assessments and develop individualized service plans for clients experiencing homelessness.
- Provide ongoing case management, supportive counseling, and crisis intervention services.
- Assist clients with obtaining permanent housing, public benefits, medical care, mental health services, and employment resources.
- Maintain regular contact with clients through office visits, home visits, shelter visits, and community outreach.
- Collaborate with community agencies, landlords, healthcare providers, and social service organizations to coordinate care and services.
- Monitor client progress and update service plans as needed.
- Maintain accurate and timely case notes, reports, and documentation in electronic databases.
- Assist clients with life skills development, budgeting, and independent living goals.
- Ensure compliance with agency policies, funding requirements, and confidentiality standards.
- Participate in team meetings, trainings, and case conferences.
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field preferred.
- Minimum of 1–2 years of experience working with homeless populations, shelters, supportive housing, or related human services programs preferred.
- Strong knowledge of housing resources, public benefits, and community-based services.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in Microsoft Office and electronic case management systems.
- Ability to manage multiple priorities in a fast-paced environment.
