Our facilities function as Skilled Nursing Facilities (SNFs), offering:
- Long-term care for residents needing continuous skilled assistance and compassionate daily support.
- Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury.
- Collaborative care through a team-oriented approach involving nursing, therapy, and medical professionals working in unison.
- A varied patient demographic that includes those with orthopedic, neurological, cardiopulmonary, and post-acute care needs.
The specific patient mix may differ slightly between facilities. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life within a nurturing, patient-focused setting.
Position Overview
The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
Education, Experience, and Licensing Requirements
- Graduation from an accredited physical therapy program, holding a BSPT, MSPT, or DPT (those graduating in 2003 or later must possess an MSPT or DPT).
- Current registration/licensure in the applicable state, with an obligation to maintain an active license in good standing throughout employment.
- Preferred one (1) year of experience in post-acute care or a related environment.
- CPR certification required upon hire or to be obtained during orientation, with the necessity to keep it current throughout employment.
Specific Job Responsibilities
- Engage in community health initiatives/projects as appropriate.
- Proficient in Microsoft Word, Excel, and email communication.
- Collaborate with patients, families, and support departments to effectively address patient needs.
- Consistently demonstrate proper body mechanics.
- Make independent decisions when necessary.
- Possess knowledge of practices, procedures, and the laws and regulations governing operations in post-acute care settings.
- Implement and interpret departmental programs, goals, objectives, policies, and procedures.
- Exhibit proficiency in all competency areas, including but not limited to patient rights, safety, and sanitation.
- Maintain professional relationships with colleagues, vendors, and others.
- Safeguard the confidentiality of all proprietary and sensitive information.
- Understand and adhere to organizational policies, including those related to harassment and compliance.
- Demonstrate integrity and professionalism by following the organization’s Code of Conduct and completing mandatory compliance training.
Core Functions
- Implement physical therapy treatment plans for patients.
- Set, evaluate, and adjust realistic, measurable, timely, and functional goals.
- Supervise and assess care provided by PTAs, Rehab Aides, and PT and PTA students.
- Document care accurately and in a timely manner.
- Utilize therapy software effectively and accurately.
- Provide excellent customer service and maintain a positive attitude towards patients.
- Assist in the safe evacuation of patients.
- Exhibit reliable and regular attendance.
- Employ reasoning skills and sound judgment.
- Communicate and collaborate effectively within an interdisciplinary team.
- Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
- Read, write, speak, and comprehend the English language.
