About the Role:
As a Physical Therapist, you will play a critical role in helping patients recover from injuries, surgeries, and chronic conditions by developing and implementing personalized rehabilitation programs. Your primary goal will be to restore mobility, reduce pain, and improve the overall quality of life for individuals across diverse age groups and physical abilities. You will work closely with patients in various settings, including home health care, outpatient clinics, and orthopedic rehabilitation centers, ensuring tailored treatment plans that address specific needs. Collaboration with physicians, nurses, and other healthcare professionals will be essential to monitor progress and adjust therapies accordingly. Ultimately, your expertise will empower patients to regain independence and return to their daily activities safely and effectively.
As a Physical Therapist, you will:

Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

Applies concepts of infection control/universal precautions in performing patient care

Assure continuity of quality patient care delivered with appropriate documentation

Provide effective communication to patient/family, team members and other health care professionals

Promote agency philosophy and administrative policies

Maintain and complete all documentation within company stated guidelines.
Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Valid Physical Therapy license
-
1 year of home health experience preferred
-
CPR certification
-
Valid Driver’s license
-
Valid Auto Insurance
-
HomeCare HomeBase experience preferred
Physical Demands

Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

Must be able to lift, carry, pull, or push up to 100 lbs..
