We are searching for a full time foster care case manager to join our team in helping to enrich the lives of young people placed in foster care. In the rewarding career you will be reporting to the social services clinical director or region director while serving as the liaison between foster families, legal parents, county agencies, the court system and other stakeholders to ensure the safety, permanency and well-being of all clients placed in CONCERN foster homes.
What Do I Need?
Education and Experience
Bachelor's Level
Bachelor's Degree in Social Work and Licensed Bachelor Social Worker (LBSW) (Required)
AND
Two years of experience in social work (Preferred)
Master's Level
Master's Degree in Social Work and Licensed Graduate Social Worker (LGSW) or Licensed Certified Social Worker-Clinical (LCSW-C) (Required)
AND
Two years of experience in social work (Preferred)
Other Requirements
- You will possess a valid driver's license and able to travel and transport clients when necessary.
- You will maintain required state and federal clearances and/or credentials
- You will function as a highly resourceful team player with the ability to also be extremely effective independently.
- You will handle confidential information with discretion, adapt to various competing demands and offer the highest level of customer service and response.
What Will I Do?
- You will administer case management/support service to each client/family assigned
- You will participate in all meetings pursuant to entrusted clients/families.
- You will complete record keeping and reporting responsibilities in a timely manner as necessary for your caseloads.
- You will develop and maintain liaison relationships with pertinent agencies, schools or professional relating to your client/family caseload.
- You will connect with clients/families on a regular basis as designated by the supervisor.
- You will meet with your supervisor on a regular basis.
- You will perform on-call and office day responsibilities as needed.
- You will link up with biological families of clients as requested.
- You will provide consultation and support and will be available for essential client/family crisis interventions.
- You will represent CONCERN at administrative/judicial reviews when needed.
- You will communicate closely with all pertinent treatment team members.
- You will fill out visit forms and arrange for drivers as necessary and appropriate.
- You will supervise visits as imperative and fitting.
- You will attend required training.
Hours of Work
You will create your 8 hour schedule around your caseload
Some evenings may be required

$48,600-$59,400
Master's Level
$52,200-$63,800
