JOB DESCRIPTION
A leading home healthcare provider is seeking a Clinical Nurse Manager to join their Elmsford, NY office. This is a full-time position offering a balanced mix of fieldwork and office-based responsibilities.
<ul>
<li>Fieldwork (40%)</li>
<li>Conduct monthly visits for each case (caseload ranges from 15 to 30 clients)</li>
<li>Support admissions and perform new client assessments</li>
<li>Develop and update care plans o Supervise and mentor field staff</li>
<li>Territory includes Northern Bronx and Westchester County, with occasional coverage in Putnam, Rockland, and Orange Counties</li>
<li>Office-Based Duties (60%)</li>
<li>Observe and validate field staff competencies (new employees and annuals)</li>
<li>Review and ensure accurate case load documentation</li>
<li>Lead staff training sessions and in-service education</li>
</ul>
REQUIRED SKILLS AND EXPERIENCE
<ul>
<li>2 years of RN experience, must have experience working in Home Health</li>
<li>Excellent communication skills Team player</li>
<li>Graduate of an accredited and approved nursing program; BSN preferred</li>
</ul>
