We are looking for a dedicated Case Manager to join our team on a long-term contract basis in San Francisco, California. In this role, you will manage a caseload of tenants, providing essential support to individuals transitioning out of homelessness. This is an opportunity to make a meaningful difference by helping clients improve their quality of life and achieve stability in housing.
Responsibilities:
• Manage a caseload of 60-90 units, providing comprehensive support to formerly homeless individuals, many of whom face challenges such as mental health conditions, substance use, and medical issues.
• Conduct outreach visits upon tenant entry and regularly thereafter to assess needs and provide ongoing case management services.
• Assist tenants with maintaining housing stability through rapport-building, frequent engagement, and effective intervention strategies.
• Support tenants in navigating benefits systems and ensuring timely rent payments.
• Address unit habitability concerns and participate in monthly pest control and unit inspections alongside hotel management.
• Organize and lead community events, tenant groups, and activities to foster a supportive and engaging environment.
• Refer tenants to appropriate services, including employment programs, and provide guidance to ensure successful follow-through.
• Respond to tenant-related crises, offering de-escalation strategies and emotional support during high-stress situations.
• Maintain detailed and confidential case management files, ensuring accurate documentation and compliance.
• Collaborate with internal teams and external providers to advocate for tenant needs and ensure comprehensive care.

• Proven experience in case management or a related field, preferably working with vulnerable populations.
• Strong administrative skills, including data entry, filing, and record-keeping.
• Ability to conduct needs assessments and develop individualized support plans.
• Excellent communication and interpersonal skills to build rapport and effectively engage with tenants.
• Knowledge of housing retention strategies and familiarity with social services programs.
• Ability to handle crises calmly and provide appropriate interventions.
• Proficiency in maintaining confidentiality and adhering to ethical standards.
• Flexibility to attend meetings, collaborate with diverse teams, and adapt to changing tenant needs.
