Systems Unlimited is currently looking to hire a Full Time Medical Care Coordinator for the Iowa City area. As a Medical Care Coordinator you will assist Systems Unlimited staff in delivering safe and appropriate health related care to individuals served by SUI. They will create educational resources and provide training to staff, along with facilitating communication with medical professionals and assisting in advocating for individuals served.
This position will be based in Iowa City and will be working primarily regular office hours (Mon-Fri 8:00 am - 4:30pm) with occasional night and weekend hours. Travel to outlying service areas is occasionally required.
Key ResponsibilitiesESSENTIAL JOB DUTIES: 1. Develop staff education and training resources for health related subjects including, but not limited to, medication administration, diabetic interventions, infection control, safety precautions, and general medical guidelines.
2. Develop and train staff on Medical Care plans to address daily health maintenance needs.
3. Advocate and act as a liaison between staff and outside medical professionals in a manner that promotes professionalism and teamwork.
4. Facilitate hospital discharges where medical care needs are changing.
5. Provide tools and trainings to assist staff in tracking interventions (i.e. BM tracking, urine output, fluid intake, etc) when needed.
6. Maintain a spreadsheet with information tracking individuals served medical cares, plans, other medical needs, etc.
7. Complete other duties as assigned.

Position and Environmental Context1. The position entails a high degree of accountability and consequence of error. Errors in judgment can result in severe consequences to the agency.

3. Certain position requirements occur in individual homes. The ability to travel extensively throughout all service areas is required.
Skills, Knowledge and ExpertiseQUALIFICATIONS:
- A background in the medical field, and/or prior work as a healthcare provider or in a health care setting.
- At least one year of experience in the delivery, planning, coordination, or administration of health or human services.
- Experience preferred in one or more of the following healthcare settings: nursing home, hospitals, assisted living, home health, medical clinic, etc.
- A valid driver's license and a driving record that is acceptable from a risk management perspective.
