About Our Facilities
Our organization operates as Skilled Nursing Facilities (SNFs) that offer:
- Long-term care: Providing ongoing skilled support and compassionate daily assistance to residents
- Short-term, sub-acute rehabilitation: Assisting patients in their recovery from surgery, illness, or injury

As a therapy professional, you will play a vital role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-centered environment.
Key Responsibilities
- Deliver direct rehabilitation care in physical therapy to patients based on physician referrals, adhering to all relevant laws, regulations, and organizational standards
- Participate in community health initiatives and projects as needed
- Maintain proficiency in Microsoft Word, Excel, and email communications
- Act as a liaison with patients, families, and support departments, demonstrating knowledge of practices, procedures, and regulations governing post-acute care facilities
- Implement and interpret departmental programs, goals, objectives, policies, and procedures
- Exhibit proficiency in all competency areas, including patient rights, safety, and sanitation
- Ensure confidentiality of all proprietary and sensitive information
- Adhere to company policies, including those related to harassment and compliance
- Demonstrate integrity and professionalism by following the organization's Code of Conduct and completing required compliance training
- Follow established physical therapy treatment plans for patients
- Supervise and assess the care provided by Physical Therapist Assistants (PTAs), Rehabilitation Aides, and PT and PTA students
- Utilize therapy software accurately and effectively
- Provide excellent customer service and maintain a positive attitude towards patients
- Communicate effectively in English, both verbally and in writing
Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must possess an MSPT or DPT)
- Preferred: One year of experience in a post-acute care or related setting
