We are currently seeking a bilingual Patient Care Coordinator (PCC) that reside in either Grand Junction or Colorado Springs, CO to join our Patient Access team! In this role, you will be the first point of contact for many of our patients, providing empathetic, nonjudgmental support in both English and Spanish, in a call-center environment. Youll schedule appointments, answer patient questions, gather essential intake information, and work closely with our clinical teams to ensure a smooth transition into care. The ideal candidate is patient-focused, detail-oriented, and confident in communicating with diverse populations, including those impacted by addiction, housing insecurity, and other systemic barriers to care!
As a bilingual PCC, you will play a critical role in expanding access to care, reducing call wait times, and ensuring that our Spanish-speaking patients feel seen, heard, and respected from the very first interaction.
Responsibilities:
- Answer inbound calls from prospective and existing patients in both English and Spanish, providing compassionate, clear communication
- Schedule, reschedule, or cancel appointments using our internal systems, ensuring provider availability and continuity of care
- Conduct new patient intake, including gathering demographics, insurance information, and consents
- Verify and update patient information with accuracy and attention to detail
- Provide continuous education to patients about what to expect during their appointments, including required paperwork, UA requirements, and clinical procedures they may be impacted by
- Support patients in understanding their financial options by referring them to financial counseling support or our membership plans and, when applicable, assisting with the initial steps of Medicaid enrollment, while maintaining an organized, patient-centered mindset.
- Coordinate care by relaying messages to clinical teams and escalating urgent concerns to appropriate staff, such as medical, behavioral health, management, etc.
- Maintain documentation standards for each call in accordance with HIPAA and organizational guidelines
- Assist with outbound follow-up calls to patients who missed appointments, need additional information, or submitted web forms for additional support or information
- Collaborate with internal teams to improve scheduling workflows, access to care, and the overarching patient experience
- Uphold Porch Light Healths commitment to low-barrier, high-access care by treating every patient with the empathy and respect they deserve
Shift: Day-time and afternoons
Qualifications:
- Bilingual in both English and Spanish with excellent verbal and written communication in both languages
- Prior experience in a medical, behavioral health, or call center setting strongly preferred
- Strong interpersonal skills with the ability to work with patients from diverse backgrounds, including those affected by substance use, housing insecurity, or trauma
- Comfort working in a fast-paced environment handling high call volume with professionalism and patience
- Familiarity with EMR and healthcare software; Salesforce and Five9 a plus
- High School diploma or equivalent
- One (1) year customer service
- Knowledge of medical terminology, insurance and billing
- Must Reside in either Colorado Springs or Grand Junction Colorado
