Job Summary
As an Intake Assessment Counselor/Case Manager, you will play a crucial role in supporting our clients by conducting comprehensive assessments, providing counseling and guidance, and coordinating the delivery of essential services. Your expertise and compassion will be instrumental in helping our clients navigate the challenges they face and access the resources they need to improve their well-being.

Key Responsibilities
- Conduct thorough intake assessments to evaluate the needs and circumstances of each client
- Provide one-on-one counseling and support to help clients identify their goals and develop personalized action plans
- Coordinate the delivery of a range of services, including housing assistance, employment support, and access to community resources
- Maintain detailed case notes and documentation to ensure the continuity of care for each client
- Collaborate with a team of social workers, case managers, and community partners to deliver holistic support
- Advocate for clients and navigate complex systems to secure the necessary services and support

Qualifications
- 2-3 years of experience in a case management or social services role
- Strong interpersonal and communication skills, with the ability to build rapport and empathize with clients
- Proficiency in assessing client needs, developing action plans, and connecting clients with appropriate resources
- Excellent organizational and time management skills to juggle multiple cases effectively
- Compassionate and non-judgmental approach to working with diverse populations

Working Conditions
This is a temporary, full-time position with a flexible schedule that may include some evening or weekend hours to accommodate client needs. The work environment is typically in an office setting, with occasional visits to client homes or community partner locations. The role may involve some light physical demands, such as transporting supplies or assisting clients with mobility.
