NOW HIRING: Social Worker
Responsibilities:Conduct comprehensive psychosocial assessments of residents to identify their emotional, social, and environmental needs.
<ul>
<li>Develop individualized care plans in collaboration with interdisciplinary teams to address residents' social and emotional concerns.</li>
<li>Provide counseling and emotional support to residents and their families, especially during times of transition or crisis.</li>
<li>Coordinate discharge planning and arrange for appropriate community resources and services for residents transitioning to other care settings.</li>
<li>Advocate for residents' rights and ensure their voices are heard in care planning meetings.</li>
<li>Collaborate with healthcare professionals, including nurses, physicians, and therapists, to ensure holistic care for residents.</li>
<li>Document assessments, interventions, and progress notes accurately in resident records.</li>
</ul>
Qualifications:
<ul>
<li>Current licensure or eligibility for licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in New Jersey.</li>
<li>Previous experience in a nursing home or long-term care setting preferred.</li>
