Requirements to apply:

• Bachelor's, Master’s, or Doctoral degree approved by the California Board of Physical Therapy

• Licensed to practice as a Physical Therapist in the state of California

• Current BLS and First Aid certification

• At least 1 year of clinical experience with older adults preferred

• 2–5 years of professional PT experience in a healthcare setting (excluding internships) preferred

This is a full-time, Monday–Friday position for a licensed Physical Therapist working with the geriatric population in a community-based Program of All-Inclusive Care for the Elderly (PACE), providing comprehensive rehabilitative services, chronic disease management, and mobility support to help frail seniors remain independent and safely age in place.________________________________________

Job Overview:

As a Physical Therapist, you’ll play a key role on an interdisciplinary team that provides comprehensive, person-centered rehabilitation services for older adults. You will evaluate participants, develop individualized treatment plans, and implement therapy interventions designed to restore or maintain optimal physical functioning. The ideal candidate is skilled in delivering both group and individual therapy, collaborating with care teams, and educating participants and caregivers. This role includes occasional home visits and oversight of therapy assistants.
