<li>Start Date:&nbsp;Provider must specify what date in April they are available to start</li>
<li>Schedule:&nbsp;Monday–Friday, Four 10-hour shifts per week, 7:00 a.m. – 3:00 p.m.</li>
<li>No call / No weekends</li>
<li>Any dates unavailable in the next 3–6 months</li>
<li>Any holidays in the next 3–6 months the provider is willing to work</li>
<li>Practice Model:&nbsp;Mix of supervision and independent practice</li>
<li>Majority Case Types:&nbsp;General Surgery, Ortho, GI, Robotics</li>
<li>EMR: EPIC</li>
<li>Certifications: FL license, active out of state drivers license, NBCRNA, ACLS, BLS, PALS, BC</li>
<li>Credentialing: 120 days</li>
<li>Out-of-state providers&nbsp;only</li>
<li>Must have a&nbsp;100% clean file</li>
