About the Role Case Manager
TheCase Managerplays a vital role in supporting and empowering individuals to improve their quality of life. This position involves coordinating care, linking clients to community resources, and guiding them through personal and behavioral challenges to promote stability and growth.
What Youll Be Doing
- The role will involvecoordinating services between HRMH and partnering agencies, supporting survivors, and helping facilitate communication and care coordination. Agencies, Urban League, YWCA, Charter Schools
- The Case Manager willserve as a liaisonbetween clients and partnering agencies to ensure continuity of care and smooth service coordination
- Conduct client assessments to identify individual needs and develop personalized care plans
- Coordinate and monitor services to ensure clients needs are met effectively
- Provide crisis intervention and emotional support during challenging situations
- Promote independence and personal growth through skill-building and mentoring
- Utilize motivational interviewing techniques to empower clients toward positive change
- Collaborate with partner agencies and community organizations to enhance service delivery
- Support workforce development efforts for clients seeking employment opportunities
- Maintain accurate documentation and progress reports in accordance with agency standards
What Were Looking For
- Must be currently resided/living in Columbus, Ohio.
- Individuals withdomestic violence experienceare strongly encouraged to apply
- Direct experience in community-based mental health services is a plus
- Willingness to workafter-school hours and weekendswhen required
- Experience inchild welfare, addiction counseling, domestic violence services, or community-based programs
- Familiarity withMedicare regulations and behavioral health services
- Strong crisis management, communication, and problem-solving skills
- Reliable transportation, validdrivers license, andcurrent car insurance
- High school diploma or GEDrequired (verification will be conducted)
What We Offer
- This is a1099 independent contractorposition.
- The position ishybrid some days will be in the community, and others will be on-site at one of our partnership facilities (exact addresses will be disclosed upon interview)
- Salary will still be based on your experience.
