Summary
The Patient Advocate Nurse serves as the bridge between clients, nursing staff, providers, and leadership to ensure exceptional medical care throughout the client's treatment journey. This role focuses on improving the client experience by proactively identifying concerns, providing education, monitoring quality of care, and ensuring timely communication between all members of the interdisciplinary team.
Working alongside the Nurse Manager and nursing staff, the Patient Advocate Nurse conducts daily site visits, performs quality assurance audits, supports medication management processes, and serves as a clinical resource for both clients and staff. The ideal candidate is compassionate, organized, solutions-oriented, and passionate about improving patient outcomes through education, collaboration, and continuous quality improvement.
Patient Advocacy & Client Experience
- Conduct daily visits to assigned treatment facilities.
- Lead client medical check-in groups to gather feedback regarding nursing care, medications, withdrawal symptoms, and overall medical services.
- Address client concerns promptly and collaborate with leadership to implement appropriate solutions.
- Serve as a liaison between clients, nursing staff, providers, and leadership.
- Advocate for high-quality, compassionate, patient-centered medical care.
- Identify trends in client concerns and recommend process improvements.
Medication Management
- Monitor medication administration practices for compliance and accuracy.
- Assist with medication reconciliation and documentation review.
- Ensure medication concerns are communicated promptly to providers and nursing leadership.
- Help identify opportunities to improve medication education and adherence.
- Support quality improvement initiatives related to medication safety.
Provider Communication
- Maintain ongoing communication with physicians, nurse practitioners, and other medical providers regarding client concerns.
- Coordinate follow-up care recommendations and provider orders.
- Ensure timely implementation and documentation of provider recommendations.
- Escalate urgent clinical concerns appropriately.
Client Education
- Provide one-on-one and group education regarding:
- Detoxification and withdrawal expectations
- Post-Acute Withdrawal Syndrome (PAWS)
- Medication purpose, dosing, side effects, and expectations
- Medication-assisted treatment (MAT)
- General health and wellness during treatment
- Reinforce education provided by providers and nursing staff.
- Develop educational materials that support client understanding and engagement.
Quality Assurance & Chart Audits
- Perform routine chart audits to ensure compliance with company policies, licensing standards, and regulatory requirements.
- Review documentation for completeness, accuracy, and timeliness.
- Identify documentation deficiencies and collaborate with nursing staff to resolve them.
- Monitor trends and provide recommendations for process improvement.
- Assist with preparation for licensing, accreditation, and regulatory surveys.
Operational Support
- Monitor Microsoft Teams communications across all treatment sites to identify and respond to medical concerns.
- Ensure urgent issues are communicated to appropriate personnel in a timely manner.
- Collaborate closely with the Nurse Manager and nursing leadership on ongoing quality improvement initiatives.
- Assist with development of nursing workflows, policies, and best practices.
- Support implementation of new medical initiatives across multiple facilities.
Collaboration
- Work closely with Nurse Manager and other staff
- Foster positive interdisciplinary communication and teamwork.
- Participate in leadership meetings as requested.
Qualifications
- Active California LVN, LPT or RN license
- At least one year of experience in detox, residentialtreatmentor related healthcare setting preferred.
- Must have andmaintaina valid California drivers license, clean driving record, andmaintainpersonal auto insurance coverage.
- Current CPR and First Aid certifications arerequired.
- Must have a current TB test or chest x-ray.
- Excellent written and oral communication skills.
- Ability to work well and quickly under pressure both individually and as part of a team.
