Position Summary &amp; Responsibilities
<ul>
<li>
Evaluate patients and develop PT treatment plans
</li>
<li>
Provide gait training, mobility, strengthening, and transfer training
</li>
<li>
Monitor progress and adjust interventions as needed
</li>
<li>
Collaborate with interdisciplinary care team
</li>
<li>
Complete timely, accurate documentation
</li>
</ul>
Requirements
<ul>
<li>
Certification in Physical Therapy
</li>
<li>
Active California PT license (required)
</li>
<li>
SNF or rehab experience preferred
</li>
<li>
Strong clinical and communication skills
</li>
