Position Title:

Geriatrician / Mobile Physician
Location: Ventura County, CA (including Camarillo, Oxnard, Ventura, Thousand Oaks, and nearby areas)

Key Responsibilities:

• Conduct in-home and on-site visits to patients in assisted living, independent living, and memory care communities

• Diagnose and manage chronic and acute conditions in geriatric patients

• Provide personalized, relationship-based care with an emphasis on dignity and quality of life

• Collaborate with families, caregivers, and interdisciplinary care teams

• Supervise and provide clinical guidance to Advanced Practice Providers (NPs and PAs)

• Maintain accurate, timely documentation and ensure continuity of care

• Participate in quality improvement initiatives and support the growth of our local geriatric programs

Qualifications:

• MD or DO, Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics

• Minimum 2 years of clinical experience; geriatric care preferred

• Passion for home-based care and underserved senior populations

• Strong communication and clinical decision-making skills

• Ability to thrive in a mobile, autonomous care model

• Leadership experience a plus (Medical Director opportunities available)
