Cooperidge Consulting Firm is seeking a Physical Therapist (PT) for a top Healthcare client.

This key clinical role manages the complete therapeutic care process for an assigned patient caseload, working in coordination with the physician and the entire patient care team. The PT conducts thorough patient assessments, develops and implements individualized care plans focused on goal achievement, and directs purposeful therapy to support patients throughout the continuum of care and ensure a positive patient experience.

Job Responsibilities

  • Provide comprehensive patient evaluations and administer specialized treatment interventions within the scope of practice and professional code of ethics.
  • Assess and evaluate the therapeutic, rehabilitative, and functional status of patients to contribute to the development of the total plan of care.
  • Deliver purposeful and skilled therapeutic care (often at the patient's home, based on typical PT duties) using established procedures consistent with the physician's plan of care.
  • Instruct the patient, family, and caregiver in the safe and effective use of at-home therapies, exercises, therapeutic appliances, and/or prosthetic devices.
  • Prioritize the assigned patient caseload, determine daily staffing needs, and coordinate assignments or handoffs to maintain workflow efficiency.
  • Document all assessments, treatment plans, progress notes, and patient education accurately and in a timely manner.

Requirements

Education

  • Bachelor's Degree in Physical Therapy is REQUIRED.
  • Graduation from an accredited Physical Therapy program is required.

Experience

  • New Graduates are accepted; prior professional experience is not required.

Certifications/Licenses

  • Current State Licensed Physical Therapist is required.
  • Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of employment start date.
