Position Summary
The Housing Stability Services Case Manager provides community-based case management and supportive services to individuals participating in the Housing Support Services (HSS) program. This role helps participants maintain stable housing, connect with community resources, and work toward personal goals using a Housing First, trauma-informed, and client-centered approach.

Key Responsibilities

  • Provide in-person case management services in participants' homes and throughout the community.
  • Build positive relationships with participants and encourage engagement in voluntary supportive services.
  • Complete assessments, service plans, crisis plans, and required documentation within agency timelines.
  • Connect participants to community resources, including healthcare, employment, education, transportation, and other support services.
  • Support participants in understanding lease agreements and maintaining housing stability.
  • Respond promptly to participant needs and accompany participants to appointments when appropriate.
  • Assist participants with transportation and other barriers that may impact their goals.
  • Collaborate with team members to coordinate services and ensure participant needs are met.
  • Maintain accurate and timely records in the electronic health record system.
  • Carry and administer Narcan as trained and required.
  • Participate in supervision, team meetings, and required trainings.

Qualifications & Skills

  • Bachelor's degree required.
  • Lived experience with homelessness strongly preferred; at minimum, one year of direct service experience working with individuals experiencing homelessness.
  • Previous case management experience required.
  • Knowledge of Housing First, trauma-informed care, motivational interviewing, and client-centered service delivery.
  • Strong communication, organizational, and documentation skills.
  • Ability to build rapport with individuals from diverse backgrounds.
  • Valid driver's license and ability to travel within the community as needed.
