MUST HAVES:
- Associate’s Degree or equivalent years of relevant work experience (active allied-health certification will be considered in lieu of degree)
- 1+ years of clinical experience in nursing, social services, or healthcare field (discharge planning, case management, care coordination, and/or home/community health experience)
PLUSSES
- Medicaid and/or Medicare managed care experience
- Clinical or Care Management certification, including but not limited to: LPN, CAN/STNA, Med Assistant, Med Tech, Pharm Tech, EMT, Radiology Technician, Dietician
Day to Day:
An employer in the north Atlanta, GA area is seeking a Care Navigator for a contract to possible hire opportunity. They will be reporting up to a Care Manager through the Clinical Operations organization of the GA market and be responsible for participating as a member of the inter-disciplinary Care Coordination Team to coordinate care for members, meeting their individual needs and the needs of the population. Their main responsibility will be to engage with the member in a variety of settings to establish an effective, professional relationship, as well as participate in the identification of the individual’s needs and prioritize efforts in collaboration with the member and caregivers. Settings for engagement can include hospital, provider office, community agency, member’s home, telephonic or electronic communication. A large portion of their role will consist of educating the members about treatment options, community resources, insurance benefits so that timely care decisions can be made. They predominantly will be helping neonatal related members in support of the company’s Planning for Healthy Babies program. This is a mobile position, meaning that regular travel to different work locations is essential, and candidates can expect a 50/50 WFH/onsite schedule throughout the week
