Job Summary:
The Social Services & Admissions Coordinator is responsible for coordinating the psychosocial, emotional, and discharge planning needs of residents while also managing all aspects of the facilitys admissions process. This role ensures residents and families receive compassionate support, accurate information, and timely assistance throughout the admission, stay, and discharge process. The Coordinator serves as a liaison between residents, families, healthcare providers, referral sources, and internal departments to promote smooth transitions, maintain regulatory compliance, and support the overall well-being and satisfaction of residents. This position requires exceptional organizational skills, strong communication, and a comprehensive understanding of federal, state, and facility requirements related to social services, admissions, and resident rights.
Duties and Responsibilities
- Maintains compliance with current federal and state regulations, professional standards, and makes recommendations on changes in policies and procedures to the administration.
- Assist billing with completing Medicaid applications and renewals.
- Receives residents Medicaid card, keeps cards on file, and sends cards to appropriate providers.
- Assist billing with Maintaining residents personal fund account and providing quarterly account statements to residents and/or family members.
- Mails out care plan meeting notices to family members quarterly. Informs residents of their care plan meeting date and time.
- Informs the resident and their family of personal rights. Distributes a copy of residents rights upon admission and ensures rights are available at all times.
- Completes discharge planning for residents. Assists with referrals for home health, Durable Medical Equipment (DME), etc., implementing social care plans, and resident assessments.
- Records and maintains quarterly social service progress notes indicating response to the plan of care and adjustment to the nursing home environment. Completes social service assessment upon admission to the facility and updates yearly or when a significant change in residents condition occurs.
- Completes assigned sections of the residents Minimum Data Set (MDS) assessment quarterly and annually.
- Completes psychosocial assessments on residents upon admission.
- Schedules provider in house visits including dental, vision, audiology, and podiatry for residents.
- Assists residents and family members in obtaining necessary appliances for orthopedic, hearing, optical, etc. purposes.
- Assist activities with shopping for and with residents for needed items, using resident fund accounts or cash provided by family. Itemizes receipts and change boxes for the financial department.
- Contacts family members regarding any needs their resident may have about clothing, haircuts, snacks funds, cigarettes, etc.
- Completes bed hold forms for residents leaving the facility for hospitalizations and overnight leave of absence.
- Refers resident families to appropriate agencies when the facility cannot provide services needed by the resident.
- Completes transfer forms for residents leaving the facility for hospitalization.
- Arranges transportation to other facilities.
- Participates in team care plan meetings.
- Completes follow up calls for discharged residents.
- Assist activities with ensuring residents receive their mail.
- Manages the entire admissions process from referral to room placement.
- Receives and reviews referrals from hospitals, families, and community agencies; verifies medical necessity, payer source, and bed availability.
- Coordinates with nursing, therapy, dietary, financial office, etc to ensure readiness for new admissions.
- Performs all other job-related duties as requested.
Education/Qualification Requirements:
High School Diploma or Equivalent is required.
LPN License in the state of Georgia preferred.
Current BLS certification. (Must obtain within 90 days of hire).
Experience:
Previous clinical experience in social services and admissions required.
Language Skills:
Ability to read and communicate effectively in English, both verbally and in writing.
Additional languages preferred.
