Role of the Primary Care Physician (PCP)
The PCP in our organization is expected to:
- Be accountable for outcomes: The PCP takes responsibility for clinical care and cost-effectiveness for each patient in their panel, which can include up to 450 patients. They recognize their ability to significantly impact patient outcomes by fostering trusting relationships and encouraging behavior changes.
- Act as a health coach: The PCP guides patients in setting both short-term and long-term health objectives, collaborates with them to achieve these goals, and regularly follows up to ensure progress towards improved health.
- Simplify for action: The PCP prioritizes tasks to make behavior changes more achievable for patients and leads their care teams towards meeting performance goals.
As an organization focused on outcomes and value, we regularly assess metrics such as patient admissions per thousand and CGCAHPS (patient experience) to support PCPs in achieving success and partnership status. Each PCP will have specific targets for these metrics and will collaborate with center and market leadership, as well as their care teams, to meet these goals. A positive culture is vital in our medical centers, and PCPs are expected to promote a culture of love, accountability, and passion alongside center leadership.
The PCP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals. They will closely coordinate with managers and medical directors to ensure adherence to guidelines and participate in risk and quality management programs, clinical meetings, and other necessary gatherings that support patient health and organizational objectives. The PCP will comply with departmental goals, performance standards, regulatory requirements, and policies.
Essential Job Duties and Responsibilities:
- Provide independent care for patients with both acute and chronic conditions, particularly in older adults.
- Take full responsibility for patient care and outcomes, seeking specialist consultations when necessary while remaining involved in the patient's detailed care.
- Engage with hospitalists whenever their patients are hospitalized, regardless of the hospitalist's affiliation.
- Conduct assessments, diagnoses, treatments, management, education, health promotion, care coordination, and documentation for patients with complex health needs.
- Lead a care team that includes a medical assistant, care facilitator, and care coordinator for patients who can visit the office.
- For patients unable to visit the office—such as those in hospitals, skilled nursing facilities, long-term care, or homebound—the PCP will collaborate with transitional care teams and other available resources.
- Play an active role in managing their center and assist in covering for other providers as needed. Each PCP is also expected to contribute to patient recruitment for the center and the organization.
- Perform additional duties as assigned by management.

- Proficient in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, as well as other relevant software.
- Willingness to travel locally as needed, and occasionally regionally or nationally for training or recruitment.
- Fluency in English, both verbal and written; some positions may require proficiency in additional languages, which will be specified during the application process.
- This role requires the exercise of independent judgment.
Education and Experience Criteria:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required.
- Must possess or be able to obtain a State Medical License for the state(s) of employment.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; board eligibility is required.
- Once board certified, the PCP must maintain certification through necessary MOC, CME, and/or retaking board exams as required.
- A current DEA number for controlled substances (schedules II-V) is required.
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required within the first 90 days of employment.
