KEY RESPONSIBILITIES

• Coordinate care and referrals across medical, behavioral health, and community resources.

• Assist patients with documentation and navigation of health services and chronic condition education.

• Promote patient self-management and goal setting through proactive communication and support.

• Conduct follow-ups on abnormal labs, missed referrals, non-compliance, and other care gaps.

• Perform pre-visit planning by reviewing charts for missing immunizations, screenings, lab results, consults, and community service needs.

• Contact patients prior to visits to review care steps and ensure preparation (medications, records, labs).

• Document all patient interactions in the Electronic Health Record (EHR) accurately and timely.

• Collaborate with HIM/Referrals staff to retrieve necessary records and reports.

• Participate in interdisciplinary team meetings and contribute to continuous quality improvement efforts.

• Conduct occasional home visits based on care needs.

• Monitor the patient portal and respond to messages throughout the day.

Required Attributes & Qualifications:

• Licensed Practical Nurse (LPN) or Medical Assistant (MA) required

• CPR certification required

• Minimum 1-2 years of experience in a clinical or care coordination setting preferred

• Familiarity with PCMH principles and standards (e.g., team-based care, population health, care management) preferred

• Proficient in using Electronic Health Records (EHR) systems, particularly Epic

• Strong organizational and problem-solving skills

• Excellent verbal and written communication, including cross-cultural communication

• Bilingual in English and Spanish or Haitian Creole preferred

• Ability to work independently and collaboratively with diverse care teams.

• Knowledge of HIPAA, Joint Commission, and OSHA compliance requirements preferred
