Job Summary:
Provides direct patient care to residents in a long-term care setting under the supervision of a licensed nurse. Assists with activities of daily living (ADLs), monitors patient well-being, maintains accurate documentation, and supports a safe, clean, and comfortable environment while delivering compassionate, resident-centered care.
Job Requirements:

Active Certified Nursing Assistant (CNA) certification in the state of practice.

Current BLS certification (if required by facility).

Long-term care (LTC), skilled nursing facility (SNF), or rehabilitation experience preferred.

Ability to work evening or night shifts, weekends, and holidays as required.

Experience with PCC charting or willingness to learn preferred.

Excellent communication, teamwork, and patient care skills.
Essential Functions:

Assist residents with bathing, dressing, grooming, toileting, feeding, transferring, and other activities of daily living.

Measure and document vital signs, intake/output, and patient observations as directed.

Monitor residents for changes in condition and promptly report concerns to the nursing staff.

Maintain accurate and timely patient documentation in the electronic medical record.

Assist with patient mobility using proper safety techniques and equipment.

Maintain a clean, safe, and organized patient environment while following infection control policies.

Provide compassionate, respectful, and resident-centered care while protecting patient dignity and privacy.

Communicate effectively with nurses, physicians, therapists, residents, and family members.

Perform other job-related duties as assigned.
