<li>Assess nature of clients' situations by interviewing and reviewing personal history</li>
<li>Develop&nbsp;and execute individual care&nbsp;plans</li>
<li>Determine appropriate milestones to gauge client progress</li>
<li>Communicate client&nbsp;updates to all relevant parties</li>
<li>Facilitate referrals to other healthcare professionals and programs</li>
<li>Maintain accurate client documentation</li>
</ul>
