Join our nursing team as an MDS Coordinator! In this role, you will be involved in the Patient Assessment Process through the completion of pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). You will also participate in developing individualized patient Care Plans, identifying the needs of the patient and projected outcomes as required by Federal and State regulations.


Requirements:

  • Current, unencumbered license to practice as an RN or LVN in California
  • Current and valid CPR certification
  • Strong leadership, communication, and interpersonal skills.
  • Develop and maintain a good working rapport with interdepartmental personnel, as well as other departments within the facility to assure that the department services and activities can be properly maintained to meet the needs of the patients.
  • Previous experience as an MDS nurse
  • At least 1 years nursing supervisory experience in the provision of direct care to multiple patients/residents; previous experience in a Skilled Nursing environment
  • Experience with PointClickCare
