The Primary Care Physician (PCP) within our organization is expected to:
- Take Responsibility for Outcomes: The PCP is accountable for delivering strong clinical care and cost-effective outcomes for a panel of up to 450 patients. They recognize the significant impact of building trusting relationships and facilitating behavior changes on patient outcomes.
- Act as a Health Coach: Rather than merely consulting on illnesses, the PCP helps patients establish both short-term and long-term health objectives, collaborates with them to achieve these goals, and consistently follows up to ensure progress.
- Simplify for Action: The PCP prioritizes tasks effectively to make behavior changes more achievable for patients and guides their care teams towards meeting performance goals.
As a value-driven, outcomes-focused organization, we regularly assess various metrics for the PCP’s patient panel to support their success and advancement to partnership status. These metrics include patient admissions per thousand, utilization of 18-21 appointment slots daily (with new patients counting as two slots and follow-ups as one), CGCAHPS (patient experience), closure of clinical gaps, and medical cost measures. Each PCP will have specific targets for these metrics and is expected to collaborate with center and market leadership, as well as their care teams, to achieve them. Fostering a positive culture of love, accountability, and passion is crucial, and PCPs are seen as leaders within our centers.
The PCP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals. They will closely coordinate with relevant managers and medical directors to ensure adherence to guidelines and participate in risk and quality management programs, clinical meetings, and other necessary gatherings that promote patient health and organizational objectives. The PCP will comply with departmental goals, performance standards, regulatory requirements, and policies.
KEY JOB DUTIES/RESPONSIBILITIES:
- The PCP independently manages care for patients with both acute and chronic conditions, particularly in older adults.
- They take full responsibility for patient care and outcomes, seeking specialist consultations when necessary while remaining involved in the patient's comprehensive care.
- Engagement with hospitalists is expected whenever their patients are hospitalized, regardless of the hospitalist's affiliation.
- The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with complex health needs.
- They lead a care team that includes a medical assistant, care facilitator, and care coordinator for patients who can visit the office.
- For patients unable to attend the office—such as those in hospitals, skilled nursing facilities, long-term care, or homebound—the PCP collaborates with the transitional care team and other available resources.
- The PCP plays an active role in managing their center and may cover for other providers as needed. They are also expected to assist in recruiting patients and additional providers for the organization.
- They supervise, collaborate with, or work alongside an Advanced Practice Practitioner (APP) and remain accountable for the APP's actions while employed.
- Other duties may be assigned at the manager's discretion.

- Proficiency in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, as well as other relevant software.
- Willingness to travel locally as needed, and occasionally regionally or nationally for training or recruitment purposes.
- Fluency in English, both verbal and written; some positions may require proficiency in additional languages, which will be specified during the application process.
- This role necessitates the exercise of independent judgment.
- Flexibility to work evenings, weekends, and/or holidays as required.
EDUCATION AND EXPERIENCE REQUIREMENTS:

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required.

A current, active State Medical License is necessary, or the ability to obtain one for the state(s) of employment.

Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; board eligibility is required.

Upon board certification, the PCP must maintain their certification through necessary MOC, CME, and/or retaking exams as required.

A current DEA number for controlled substances (schedules II-V) is mandatory.

Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required within the first 90 days of employment.
SALARY RANGE:
$221,141 - $315,915
The stated salary range reflects the base hourly rate or annual salary for this position. Final compensation will be influenced by various factors, including experience, education, geographic location, and other relevant considerations. This role may also qualify for bonuses or commissions.
