Scope
The Hospice Certified Nursing Assistant (CNA) plays a pivotal role in providing compassionate care to terminally ill patients in their homes, ensuring comfort, support, and dignity in their final stages of life. This position requires a highly empathetic, patient-focused approach, working under the supervision of the Hospice Nurse Manager and in collaboration with the interdisciplinary hospice care team.
Expectations
- Patient Care: Provide direct care to patients as outlined in their care plan, including personal care, hygiene, nutrition, and mobility assistance. Ensure a comfortable and supportive environment for both patients and their families.
- Compassionate Support: Offer emotional support to patients and their families, demonstrating sensitivity and understanding of their needs and preferences during end-of-life care.
- Clinical Tasks: Assist in monitoring vital signs, reporting any changes in patient conditions to the nursing staff, and contributing to the patients' pain management and comfort measures.
- Collaboration: Work closely with the interdisciplinary team to implement the care plan effectively, participating in meetings and providing input based on direct observations and patient interactions.
- Education and Advocacy: Educate families on basic care techniques; advocate for the patient's comfort and family's peace of mind.
- Documentation: Maintain accurate, timely, and confidential patient records, documenting care provided and observations of patient status.
- Professional Development: Participate in ongoing education and training to enhance knowledge and skills in hospice care practices, adhering to the highest standards of healthcare delivery.

Qualifications
- Certified Nursing Assistant certification with a valid license to practice in the state of North Carolina.
- Previous experience in hospice or palliative care is preferred but not required.
- Demonstrated ability to communicate effectively, both verbally and in writing, with patients, families, and team members.
- Strong organizational skills and the ability to work independently.
- A compassionate nature and a deep commitment to providing empathetic care to patients and their families.
- Ability to travel per job requirements.
