Location: Brooklyn, NY 11225

Schedule: Flexible, 35 hours per week

Role Type: 13-week contract with potential for extension and/or hire

Compensation: $45–$48/hr. during the contract period; $90,000–$95,000 per year with comprehensive benefits upon hire

Position: RN – CRTP/ICM

Overview:

The Registered Nurse (RN) provides comprehensive nursing care across the Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI), integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN works closely with interdisciplinary teams to ensure health, safety, and successful community integration while addressing complex medical, behavioral, and psychiatric needs during transitions from inpatient settings to the community.

Key Responsibilities (including but not limited to):

Clinical Care

  • Screen individuals for admission to CRTP and Transitional Respite Programs; actively participate in the intake process.
  • Conduct initial and ongoing nursing assessments for program participants.
  • Oversee self-medication administration, monitor therapeutic responses, and document outcomes.
  • Provide direct nursing interventions including wound care, diabetes management, respiratory support, and chronic disease oversight.
  • Monitor early signs of medical or psychiatric crises and initiate appropriate interventions.
  • Review medication administration records, vital signs, and other individualized logs; report significant issues to relevant clinicians.
  • Ensure proper medication storage and disposal in compliance with federal and state law.
  • Develop and monitor individualized health goals; update plans as needed.
  • Follow OSHA infection control and universal precaution guidelines; maintain medical supplies.
  • Provide 24/7 phone consultation for urgent care and guidance.


Case Management & Transitional Support

  • Collaborate with ICM teams to address medical and behavioral health needs in service planning.
  • Provide health education and coaching to individuals, families, and support staff to promote independence and wellness.
  • Facilitate hospital discharge and community reintegration through CTI, ensuring continuity of medical, psychiatric, and social support.
  • Coordinate follow-up care with primary care providers, specialists, and community-based resources.


Collaboration & Training

  • Serve as a clinical liaison among providers, families, and program staff.
  • Participate in interdisciplinary treatment planning and case conferences.
  • Train and mentor direct support professionals (DSPs) on health-related tasks and emergency response.
  • Maintain accurate documentation in compliance with OPWDD, OMH, and DOH regulations.


Qualifications:

  • Current, unrestricted New York State RN license.
  • Minimum 2 years of nursing experience; experience with IDD, behavioral health, crisis care, or community health preferred.
  • Knowledge of OPWDD, OMH, and DOH nursing regulations and program requirements.
  • Strong clinical assessment, crisis intervention, and care coordination skills.
  • Excellent communication, organizational, and documentation skills.
  • Ability to work independently while thriving in a team-based environment.
  • Valid driver's license preferred (travel between program sites may be required).
