Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we’re looking for a Senior Care Coordinator to join our award-winning team.
Key Responsibilities
- Calls on physicians, hospitals, skilled nursing facilities, and rehabilitation discharge planners and management within an assigned territory to promote designated service lines.
- Conducts pre-discharge visits to hospitals or other facilities at the request of physicians, and upon receiving physician orders, to make a preliminary determination of the need/eligibility for home care.
- Maintains a detailed territory list, prioritizing accounts in accordance with the market/territory sales plan.
- Maintains records in detailed territory management systems.
- Provides leadership and accountability by coaching, developing, and retaining a high-performing sales team.
- Demonstrates consistency in achieving both admission and census goals by monitoring and assessing the progress and development of each Account Executive/Territory.
- Responds promptly to any performance gaps and addresses barriers to achieving budgeted expectations.
- Compiles, communicates, and executes best practices in the field. Help develop new ideas, protocols, and efficiencies for maximizing marketing resources to grow market share.
- Develop market-specific sales strategies to generate new business.
- Build a collaborative partnership with the Operations Leadership team.
- Creates and supports a shared vision for growth between Sales and Operations.
- Drives company initiatives and brings realistic solutions to challenging situations.
- Manages Care Coordinator turnover and marketing expenses.
- Provides leadership and accountability through individual one-on-one meetings and coaching sessions.
- Retains a high-performing sales team through continued development and support.
- Quickly and effectively addresses any individual performance gaps or barriers to meeting expectations.
- Leverages data services to identify new referral opportunities and improve underperforming disciplines.
- Build a high-performing Sales/Marketing team by recruiting and hiring top sales talent.
- Performs other duties as assigned.
Qualifications
- High School Diploma or GED required
- Bachelor’s degree in a related field preferred
- Nursing background is a plus
- Minimum two years of experience in home health care
- Proven track record in business development and market growth within home healthcare
- Demonstrated ability to coach home health care sales teams to exceed performance thresholds
- Ability to travel, including some overnight travel
- Flexible schedule with availability to attend after-hours and weekend sales events in various locations
